I have a bit of a problem, my high(pitch) E is still flat at the 12th fret even after the saddle has been adjusted all the way towards the neck. Tried flipping it, still not enough.

I have never had this problem before, didn't even notice my intonation was off until this past week.

Additional information

Strings are D'Addario Nickel round wound 11-49's
Probably about 4 weeks old
I usually use Ernie Ball Power Slinky's, but my local music store only had these.

Could just be time to get some new strings? I usually never have intonation problems even with old strings.

You can have this problem with just one string from a bad batch. By all means try a new one.

I think its a dead spot on the string between the 11th and 14/15th fret. When the E and B are in tune with each other, both the 7th fret and 19th fret of the E are intonated with the B

What he said. I use diadario and tried some boomers cause they were cheap. Same gauge and threw my intonation out like crazy!! Never using boomers again.

I had this problem also. If you get a roller bridge like the Schaller or Wilkinson you can adjust out farther to get the correct intonation. Also, not cutting your hands on the Tune-O-Matic when beating the Hell out of the guitar is a big plus! I had this problem...
