My 2002 68 LPC (natural top, brown back/sides) came with one. Its a Yamano guitar also fyi

From 1991 to October of 1993 the Custom Shop was in the main factory.
In October of 1993, Gibson opened the Custom Shop in it's own building,
but the COA's followed the old Serial Numbering system.
In 1994, the Serial Numbers were changed to the format still used today.

My 2002 and 2003 ‘68RIs came with the sunburst colored COAs as pictured above (great post DEGold!).

The Flametops came with an “F” denoted in the model description. Otherwise, the COA says “‘68 Custom Auth”.
