Think about this from a construction standpoint.

If you place a truss rod anchor in the middle of a neck and then shape the neck to profile, removing the wood surrounding the anchor. When you tune up to pitch where is the anchor gonna go?

That's it - right out the back of the neck.

Sounds like the guitar in question had a warp that the TR couldn't work out - any guitar repairer worth his salt can fix this by slackening the TR all the way and clamping to a jig and heating.
